BodyActive Therapeutics - Uniquely Whole, Vital and Balanced
Policies

Professionalism
You can always expect professional behavior and treatment practices in a safe and nurturing environment. If at anytime you feel uncomfortable for any reason, please let me know immediately. This is your time, so feel free to speak up.

Confidentiality
All client information is kept strictly confidential. I will not release information to anyone, under any circumstances, unless I first obtain your express written permission or am subpoenaed by a court of law.

Draping/Etiquette
You will undress in privacy to your level of comfort and will be draped at all times. Only the area of the body being worked will be uncovered.
It is expected that you will come to the session recently showered/clean.  The sheets that are used will be changed and sanitized with each client.

Client/Therapist Relationship
These massage sessions are therapeutic and non-sexual in nature. Sexual behavior or harassment is not tolerated. If at anytime my safety is compromised the session will be stopped immediately and charged in full.

Intoxicants
Since massage increases circulation, it can accelerate the use of intoxicants. If you arrive under the influence of alcohol or any other intoxicants, the session will be forfeited and full payment will be requested at that time.

Scope of Practice
While massage enhances medical treatments and may shorten the time it takes for the body to recover from injury and illness, it is not a replacement for medical treatment. Should your condition require medical attention beyond my scope of practice, a referral will be made to a qualified medical professional. It is not within the scope of practice for a massage therapist to make a diagnosis, prescribe for specific conditions, or offer counseling.

Fee Structure
All fees are posted on my website. I will notify current clients of any fee changes one month prior to the change in the form of an e-mail. I will also inform clients of the new fee schedule at the end of their sessions. All package deals and gift certificates purchased prior to the fee change will be honored through the expiration dates at the old prices.

Payment, in the form of cash or check, is expected at the time of service. At this time credit cards are not accepted. In the case of a bounced check you will be asked to cover an additional $20 fee over the amount of the check. If this situation arises more than once, all further payments must be made in cash.

I do not offer insurance billing or reimbursement. I will provide you with a receipt to assist you in obtaining your own reimbursement from your insurance company.

Cancellation/Late Policy
You will be charged the session rate for any cancellation made within 24 hours of your scheduled appointment. This may be waived if your original appointment can be re-scheduled in the same week. Exceptions may be made for extraordinary circumstances.

If you are feeling ill on the day of your appointment, please notify me as soon as possible. We can decide together if you should keep your appointment. If you have to cancel your appointment due to illness, no cancellation fee will be charged.

When scheduling appointments, I allow for a 10-minute grace period. If a client arrives more than 10 minutes late, I will do my utmost to provide a complete massage in the time remaining. Full payment for the entire scheduled session will be expected.
